Immunization Requirements

Georgia requires students to meet applicable immunization requirements before attending school. Families must provide a valid Georgia Certificate of Immunization, Form 3231, or qualifying exemption documentation.

Students entering seventh or eleventh grade and students new to Georgia may have additional documentation requirements.

Illness and Medication at School

All medication administered during the school day must follow school procedures and applicable law. Families should contact the school before sending prescription or over-the-counter medication with a student.

• Use the required medication authorization forms.
• Provide medication in its original, properly labeled container.
• Notify the school about health conditions that may affect the student during the school day.

Seizure and Epilepsy Support

In accordance with Georgia law, The Meliora School provides seizure recognition and first-aid training to applicable personnel and maintains seizure action plans for students with diagnosed seizure disorders.

Families should contact the school to establish or update a seizure action plan.

Diabetes Care

The Meliora School maintains trained personnel to support students with diabetes and works with families to implement an appropriate diabetes medical management plan.

Families should contact the school to begin or update the planning process.

Sudden Cardiac Arrest

The school provides information concerning the signs, symptoms, and risks associated with sudden cardiac arrest as required for schools serving grades 6-12. Additional requirements may apply to student athletics and other activities.

Water Safety Education

Water safety education helps students recognize hazards and practice safer behavior around pools, lakes, rivers, and other bodies of water. Families are encouraged to discuss water safety and use qualified local swimming-instruction resources.
